Responsibilities

• Provide high-quality home-based care to patients following hospital, rehab, or nursing facility discharge
• Collaborate with physicians and supervise LPNs, CNAs, and aides
• Conduct patient assessments, document progress, and communicate changes to physicians
• Educate patients and families on care plans, medications, therapy, diet, and exercise
• Coach families on care plans, medication management, and home safety
• Ensure treatment goals are achieved and facilitate smooth patient discharge

Requirements

• Minimum 1 year of acute care experience
• Home healthcare experience and Oasis familiarity preferred
• Graduate of an accredited nursing program with an active FL RN license
• CPR certification and knowledge of state and federal home health regulations
• Strong clinical proficiency, communication skills, and compassion
