Phamily is seeking a dedicated Chronic Care Navigator to join our compassionate team. As a Chronic Care Navigator, you will play a crucial role in supporting the development of patient-centered care and working closely with our nephrologist to manage patients efficiently.

Key Responsibilities

• Understand nephrology requirements for optimal population health.
• Collaborate with care teams to establish effective care processes.
• Utilize the Phamily Chronic Care Management platform to support patients with chronic diseases.
• Contribute to quality improvement and care redesign efforts.
• Provide exceptional patient and family support through various interactions.
• Ensure compliance with policies and procedures.
• Participate in various meetings and maintain teamwork.
• Perform tasks to support care teams and patient management.
• Monitor and correct patient attribution within the practice.
• Additional duties as assigned.
